Every profession has a certain amount of paperwork, but the good news is I only require three brief documents before we can get started. You have two options: (1) submit them quickly on-line [see below]; or (2) you can print them, fill in the appropriate blanks, sign, date and fax or mail them to me (OR, I can send them to you by regular mail¾ whichever is easiest for you):
